Coronary computed tomography angiography (CCTA) is a noninvasive 3D imaging test that identifies plaque and blockages or narrowing (stenosis) of the coronary arteries. During the test, a dye is injected through an intravenous (IV) line in the hand or arm, and computed tomography (CT), a combination of X-rays and computer technology, is used to produce images from inside the body.

  • The CT scan itself will take less than 1 second! However, there is preparation, heart rate optimization and positioning involved in order to obtain the best scan. At CT Heart San Diego, we offer a full service package to have everything done onsite. On average, patients will be there for 30 minutes, but we ask to plan for a 1 hour time period as every patient’s preparation time can vary.
  • Heart rate optimization is needed to get the best picture possible with the least amount of radiation exposure possible. Because the coronary arteries are in constant motion, they are difficult to capture a clear picture of. Slowing the heart rate down can help decrease motion artifact. Although our state of the art scanner can capture the coronary arteries at almost any heart rate, it is still proven that: lower the heart rate, better the scan, lower the radiation.

  • We use short acting beta blockers to decrease heart rate. Most will receive oral beta blockers which has a half life (time of effectiveness) of approximately 6-7 hrs. Some patients may require a short acting IV dose of beta blocker to further bring the heart rate down. If you do not want beta blockers for any reason, you can forgo this portion and still get a scan completed.

Plaque quantification with artificial intelligence software is an FDA approved service that helps differentiate high risk plaque and low risk plaque. It can identify the exact composition in quantity of plaque buildup, which is important in predicting future heart attacks and for tracking disease. Quantification is especially helpful in assessing response to therapy, may that be lifestyle, modifications, supplements, or drugs. For example, you can scan a baseline CT and then repeat again in one to two years to assess your response to therapy. This is called personalized preventative cardiology.

Coronary artery disease (CAD) can often be managed and its progression can be slowed or even halted, but reversing it completely is more challenging. There are several clinical trials that have assessed the response of both lifestyle modifications and drugs on plaque progression and transformation. What has been found is that more often than not, plaque can be transformed from high risk plaque (those that cause heart attacks) to low risk plaque. While some cases may have regression, majority show stabilization and stopping progression of disease with appropriate therapy. If your study shows that you have plaque buildup in your coronary arteries, we would recommend meeting with a preventative cardiologist. Treatment can significantly improve heart health, reduce symptoms, and decrease the risk of heart attacks and other cardiovascular events.

Plaque build-up in the coronary arteries is called atherosclerosis. In order to have a heart attack, or needs stents or bypass surgery, you need to have some level of plaque. For the longest time, coronary artery disease was diagnosed after you already had a heart attack. With this test, we can detect atherosclerosis early and stop the progression that would lead to a heart attack. With new AI technology, we can track the disease progression and its response to therapy just like you would check your blood sugar or cholesterol levels. We can stop the disease progression and prevent heart attack.

For the advanced and pro package, intravenous iodinated contrast is given to visualize and evaluate the coronary arteries. It is required to check kidney function via a lab test within 60 days prior to the study. This is to evaluate how well the kidneys are working and to ensure they can safely process and eliminate the contrast material. For patient convenience and service, we check the creatine level on site with a point of care test result within five minutes. There is no need for you to make a separate trip to the lab prior to your study.

No you do not need it. The test will be done under the supervision of a cardiologist and recommendations will be given on the report. If there is a critical finding, that needs immediate intervention, a cardiology practitioner will call you and can refer you to take the correct course of action.

CT angiography and AI plaque analysis are currently not indicated per most insurance guidelines as a preventative test when patients have no symptoms. CT heart scan offers multiple tests and services bundled into the package price, therefore these particular packages will not be covered by insurance.
